A rhinoplasty is mostly designed to make a better appearance and function of the nose. All development in this area has led to a less invasive procedure that is able to create more satisfying results. The nose of each patient is unique so, an experienced surgeon chooses the most appropriate technique that suits the cosmetic nose surgery candidate needs. Closed rhinoplasty method serves as a new and less invasive technique and it is necessary for expert to have a lot of experience to be able to make nose modifications through application of this state-of- the art technique. In this method, the procedure is performed through limited inside incisions so, it is critical to search for a plastic surgeon who is skilled enough to make improvements just through a few nasal internal incisions.

What is closed-rhinoplasty?

The closed method of nose surgery as a modern and less invasive technique involves making all incisions inside the nostrils to reshape the nose and create a great facial balance.

Benefits of this rhinoplasty type

this new strategy aims to clear constraints of traditional procedures. Some of them are as follow:

preventing scar tissue : as mentioned before, in closed rhinoplasty technique all incisions are located inside the nostrils. It is a big privilege that avoids a visible external scar. It is very common among many candidates to have concerns about visible scares. If you worry about scar after cosmetic nose surgery, search for a skilled surgeon who performs closed technique without the formation of scar tissue after a nose job.

Reduced recovery : Small and limited incisions in closed rhinoplasty mean less swelling and bruising after surgery. Besides, the healing process would be faster. All these lead to shorter recovery and faster relieving of swelling and bruising.

Shorter length of operation : Using modern technique of preservation in correcting nasal problems makes it possible to perform the operation and create improvement in the nose during a shorter period of time in operating room.

Natural results : Closed rhinoplasty is a less invasive surgery that helps patient to get to the best result with less injury of the nose during the operation. Today, many people who think about a nose job, look for a result that can create a more proportionate nasal shape and restore functionality too.

Good candidates for closed rhinoplasty

Anyone who is unhappy with the appearance of their nose can be a good candidate for this procedure. This technique has specific features that make it suitable for patients with fleshy noses. Thick skin of fleshy nose is prone to swelling and scar formation. Modern preservation technique has been a fantastic option for fleshy nose patients to experience less swelling and scar formation.

It can also remove bump on the nasal bridge that is usually a prominent feature on the face of people with bony noses. This technique can be applied to correct septal deviation, too.

Closed rhinoplasty limitations

Closed technique is a delicate procedure, but like any other surgery, has some limitations, too. Since there is internal incision in this procedure, there is less alteration of nasal tissues, so significant changes can not be made during the operation. due to these limitations, this approach can not be an appropriate option for a revision rhinoplasty candidate who needs extensive changes in the nose. In revision surgery the surgeon needs to fully visualize the nasal internal structures to perform the procedure. Full visualization means making a bigger incision on the nose.

Note: The nose expert fully evaluates the patient condition to make an appropriate decision about the proper technique to make modifications.

How is modern rhinoplasty technique performed?

Consultation is the first step in your surgery procedure in which the surgeon determines your candidacy based on the information from your medical condition evaluation. This surgery is usually carried out under general anesthesia that allows you to sleep during the operation.

Internal incisions are used to lift the nose skin in the closed procedure. However, if the preservation technique is applied to perform the surgry, all modifications will be done while preserving nasal structure. After removing bump or straightening deviated nasal septum, nasal contour line will be created if needed. At the end of the operation, the incisions will be closed by surgical sutures.

Tips to do before closed rhinoplasty procedure

To prepare for closed nose surgery, you need to get prepared physically and mentally. To this purpose:

  • You are asked to schedule a consultation
  • Your surgeon evaluates your medical history and present physical condition
  • You and your surgeon discuss about your goals and expectations from closed nose surgery procedure
  • Some examinations such as blood test is prescribed

Some of pre-operative instructions are as follow:

  • Stop smoking and drinking alcohol two weeks before surgery
  • Avoid taking blood thinning medications such as aspirin. They increase the risk of bleeding during the operation.
  • Don’t eat or drink 12 hours before the procedure.
  • Problems can be treated with closed rhinoplasty

Getting a closed nose job is usually improving cosmetic and functional problems of the nose with less invasive techniques. Some of the defects that can be treated using this method are:

  1. Crooked nose result from trauma or birth irregularities
  2. Large and wide nostrils
  3. Deviated septum
  4. Nasal obstruction
  5. Tumor or abnormal tissue detection in nose

Differences between open VS closed method

When it comes to closed and open approaches, no two surgeries are the same, but in brief the main differences between the two approaches are as follow:

In closed rhinoplasty:

  • All cuts are internal
  • Recovery is faster
  • It involves less swelling and bruising

In open rhinoplasty:

  • Cuts are both internal and external. The external cut is made on the columella, the soft tissue between nostrils
  • Recovery is longer
  • There is more swelling and bruising

It is believed that less invasive closed rhinoplasty provides less access to internal parts of the nose that influences the delicacy and accuracy, but it should be reminded that modern closed  approach is an expert technique that aims to obviate limitations of traditional methods, so by performing procedure in the hands of an experienced expert, the more satisfying results can be expected. Taking time to search for an experienced surgeon who is truly able to manage challenges of the procedure, is the key to achieve a successful outcome.

Closed-rhinoplasty aftercare instruction

You may have a splint over your nose after rhinoplasty procedure. It is used to protect and support nasal structures as you begin to heal. You are not allowed to remove this splint for any reason, unless directed by your doctor.

No Smoking or Alcohol Consumption

Quitting smoking and other forms of nicotine consumption is necessary during the recommended period of time before and after the surgery. Smoking and drinking alcohol will disrupt wound healing. Do not consume any nicotine or alcohol for almost 3 weeks after surgery.

Showering

You must care your nose carefully after rhinoplasty procedure and you need to make sure your face does not get wet. It is very important to keep the splint on your nose dry at all times. After a week your surgeon will remove the splint and you can shower or take bath as usual.

Sleeping

Keeping your head elevated while sleeping is really important to avoid putting any pressure on the nose. This will reduce swelling and will be more comfortable for you. Do not sleep flat or on your stomach.

Rest, but not bed rest. It is a good idea to get up and walk under your own strength. Walk 10 minutes every 2 hours. It should be light walking indoors. Recline, do not lie down. This will be more comfortable for you, and can reduce swelling. During sleeping, keep your head elevated by putting two or more pillows under the head. Do not bend forward or over.

Relax and do not engage in any stressful activities, and do not resume any exercise routine other than walking.

Dressings

You may have gauze taped beneath your nostrils. You can change it when it is soaked with blood and drainage. This is important to hygiene.

Medication

Take all oral pain medication and antibiotics prescribed by the surgeon. For less severe pain, use over-the-counter Tylenol. Prescription pain medication can contain acetaminophen. Do not take pain killers that include blood thinner.

What to anticipate following a closed rhinoplasty

Following the procedure, you will wear a cast or splint for about 5 to 7 days to provide support to your nose during the healing process. It’s important to arrange transportation home in advance, as you will not be able to drive while under anesthesia. Expect to take a few days off from work and try to sleep with your head elevated, as this can help minimize swelling. Your surgeon may advise you to avoid the following:

Engaging in strenuous activities or exercise

Excessive chewing

Swimming

Drinking alcohol

Blowing your nose

Sleeping face down

Pulling clothes over your head

After approximately a week, the cast or splint will be removed, unveiling your newly shaped nose. While you may still experience some bruising and swelling around the nose and eyes, you can gradually resume normal activities. The results are typically permanent unless your nose is subjected to injury or trauma.

How is closed rhinoplasty performed?

Closed rhinoplasty is executed through incisions made inside the nostrils, providing access to the nose’s bone and cartilage. This surgical technique allows the surgeon to address various aspects of the nose, including the dorsum, root, lateral walls, and nasal tip. Additionally, if there is a deviated septum present, it can be corrected simultaneously with a procedure called septoplasty (or septorhinoplasty). As all cosmetic alterations are conducted without external incisions, there are no visible scars on the skin. The success rate of this facial plastic surgery can be notably high, particularly when conducted by a board-certified plastic surgeon with extensive expertise in closed rhinoplasty and septoplasty.
